Gnomon Informatics S.A. has successfully concluded its participation in the SHAPES piloting activities by adapting the eHealthPassTM digital health platform to the specific needs and requirements of the piloting themes. We are proud to have been part of such a vast and diverse ecosystem involving partners from various sectors, such as industry, academia, healthcare, and social care. Our contribution, the eHealthPass platform, played a crucial role in facilitating the piloting themes mentioned below, showcasing its versatility in handling a range of digital health scenarios, from managing chronic diseases to aiding the elderly at home and enabling international healthcare exchanges.

SHAPES Pilot theme 2: Improving In-Home and Community-based Care

This theme revolved around creating an ideal home environment for elderly individuals requiring care, emphasizing specialized nursing services. The main goal was to foster a nurturing environment that promotes autonomy without compromising the need for medical attention. Various digital solutions and smart devices were employed to monitor the inhabitant’s activities and gauge their well-being.

SHAPES Pilot theme 3: Medicine Control and Optimisation

The primary focus was to identify and enhance adherence to medications and treatments among older individuals with chronic illnesses.
